IMPORTANT UPDATE: 01/12/25
From Mon 8th December 2025 00:00hrs, the Performance and Well-being Conversation will no longer be hosted on SWAST CPD. From Mon 8th December, it is now hosted: Updated Appraisal Process – Launching 8 December
ALL staff (staff & line manager) completing a Performance and Well-being Conversation must copy and paste the content (in a draft email or word doc.) and save this to ensure you have a record. It will not be possible to retrieve any records. This is especially important if you are attempting to complete close to Mon 8th Dec 2025
Confirmation emails:
The emails that your PWBC form generates may be sent to your PMM (junk) folder. Please check your PMM folder if your email doesn’t appear in your inbox. To find this email search Outlook for “PMM Message” – open this email and click “deliver” next to the message you need to retrieve. This will then be sent to your inbox as normal.

There is a new Performance Wellbeing Conversation (PWBC) / appraisal system which you can access on a SWAST device at the following location (copy & paste into your browser):

If you are only part way through completing your PWBC e.g. your manager is yet to complete their link; they can complete this on a word document for you both to save for your records. Please just email pwbc@swast.nhs.uk when you have completed your PWBC (they do not need a copy of the paperwork).
